Partner

You cannot add your partner or children to the existing ‘Verdragspolis’. They need to take out their own compulsory general insurance policy in the Netherlands.

If you would like more information about general insurance policies from CZ, feel free to call us on +31 (0)88 555 77 77.

Package for your partner

The ‘Jongeren’ and ‘Gezinnen’ additional insurance packages already include dental insurance. If you have either of these packages, you will not need to take out separate dental insurance.

Details of your partner

The customer number is stated on your policy and your CZ Zorgpas
If your partner is not an EU/EEA national, we need a copy of the front and back of your partner’s residence permit.
We will cancel your partner’s existing general insurance policy from the date from which your partner is covered by the ‘Verdragspolis’. We will do this as we are obliged to supply your current health insurer with data required to provide health insurance and to implement the Dutch Health Insurance Act (‘Zorgverzekeringswet’). The same applies to co-registered members of your family.

Children

If your child(ren) have their own income in the Netherlands, you cannot add them to the existing ‘Verdragspolis’. In this case, you must take out a compulsory general insurance policy for your child.
