To complete your registration, please send us the following documents:

• The convention form 106, 109, S1 or 121 issued by your foreign health insurer.
• If you are not an EU or EEA national, we need a copy of the front and the back of your and any family members’ residence permit.
• If you work for an international organisation (such as NATO) or at a foreign embassy, please also send us a copy of your valid passport.
