Group interventions?

You do not need your employee’s permission for a group intervention. You can request our services immediately using the form at the bottom of this page.

Individual interventions, waiting list mediation, healthcare recommendations

If you want to use an individual intervention, waiting list mediation or a healthcare recommendation, you will need to get your employee’s permission first.

Here’s how it works:

Requesting individual intervention, waiting list mediation or a healthcare recommendation. The CZ Healthcare Coach (‘Zorgcoach’) will contact your employee to discuss the request and ask for permission, and your employee will then receive an email confirming what was discussed.
The permission form is sent to the CZ Healthcare Coach. Our coaches will initiate the request once your employee has given permission.

In accordance with the General Data Protection Regulation (GDPR), personal details concerning health will only be shared with you when strictly necessary as part of the provision of the service. You will also receive progress updates. Personal details concerning health are only shared with you or your company doctor where this is strictly necessary and permitted under the GDPR.

After completion of the intervention, you will be contacted by market research company Blauw Research representing CZ to evaluate the interventions used.
